Patient goals should do more than open a plan of care. They should actively guide how treatment progresses. Progression is informed by clinical guidelines, experience, and timing, but it should ultimately reflect what the patient is working toward in real life and when they are ready for it. A simple check to come back to: If a patient’s goal changed today, would their treatment change with it?
