In response to the CMS 1500 form transition timeline released by the NUCC, all medical service providers should stock up on the revised 02/12 CMS 1500 form.

The new CMS-1500 form version 02/12 will replace the version 08/05 and give providers the choice of using ICD-9 or 10 diagnosis codes, which will help the transition in October 2014.
